Fingerprint

Yes, download and print the FBI National Record Check Release here. Your signature of the form must be notarized and then uploaded onto the portal as set out below. When your application is received, you will get an email with a Vermont Fingerprint Authorization Certificate and a list of Vermont Fingerprint Identification Centers. Make an appointment to get your fingerprints taken and bring your certificate with you. There may be a fee to pay. The process can take several weeks.

How to

Fill in the online form

1. Create an account on the Office of Professional Regulation portal
2. Log in and click on the list icon in the top left corner
3. Click on “Apply for a license”, then “license”
4. In “Profession” select “Nursing”
5. In “Profession type” select “Licensed Nursing Assistant”
6. In “how you are applying for licensure” select either “endorsement”, or if you have been licensed in another state for 3 or more years, select “fast track endorsement”
7. Normal endorsement route:
   • Fill in the “Applicant Information” and the “Address Information”
   • Answer all of the questions in “Good Standing”
   • Answer all of the questions in “Mandatory”
   • Answer all of the questions in “Fitness”
   • Answer all of the questions in “Profession Specific”
   • In “Profession Specific”:
     • Download the form (also available here), fill in the first box, sign and date
     • You can request a provision license at this stage
   • In “Qualification”:
     • If you previously clicked “Yes” on having completed a Nursing Assistant Program within the last two years, enter your Nursing Assistant/LNA Certificate Program Information in “Education Details”
     • If you previously clicked “Yes” on having worked as a Licensed Nursing Assistant for paid compensation for 50 days (400 hours) under the supervision of a licensed nurse for the last two years, enter your employment information for the last two years that show you meet the experience and supervision requirements in “Experience”
     • enter all states in which you have or ever held a license
   • Upload your signed and notarized FBI National Record check Release Form (see above)
   • Review the Summary to ensure all the information is correct
   • Read the Attestation and click “I Agree”
   • Pay the fees and submit your application
8. Fast Track endorsement route: the application is the same, and you must fill in, sign and notarize the Verification of Licensure form, and submit a signed and notarized FBI National Record check Release form. The only difference is that you do not need to provide any information regarding Qualifications
